How staying out of the USA affects the Green Card status

My husband and I (with British passports)have been living in the USA since 1991 ( 17 yrs approx)We have been Green Card holders since then and during that time we have worked, run a business and paid regular taxes like any American citizen. Last July due to the economic crisis in the USA we decided to retire and have been travelling since then to Central America to see if we can afford to live for most of the year in a Country that is less expensive. We have great difficulty living in the US with our meagre pension.
We have a home in the USA and my husband is on Medicare and I will be 65yrs old in June and also will be on medicare. Both of us receive small USA pensions. We have been paying taxes every year to the USA tax authorities including 2008 return. We own a property in the USA. We both have valid USA drivers licences. We both have USA bank accounts. I would like to know the following:

a How long can we remain out of the USA without losing our Green Card and residency status.?
b.Is our Medicare status at risk?
c.If we have a medical emergency can we return for treatment?

We have 2 daughters living in the USA and our intention is to stay out of the USA (again not necessary by choice but being able to afford to live) and we would like to return at least once or two times a year to visit them.

I am very concerned and need advise. Do you recommend we apply for USA citizenship and if so would this eliminate the problem.

You can search for "H1B Visa Employers" using this database, which was compiled by Antao & Chuang, Attorneys at Law from government sources. This database identifies those U.S. employers who have filed for H-1B visas in the past, or who have at least started the process by filing for the LCA. If you find an employer you are interested in, you can then contact them to inquire as to whether they have any current job openings in your field. Foreign Medical Graduates ("FMGs") should be aware that there are "FMG Friendly" employers, and "FMG Unfriendly" employers.
